weird screen after i respawn

so i was in the ice cave and got killed, idk if its the ice effect or the game not knowing where to put me or some other problem, i was also in the entrance area. so maybe thats the problem? idk, but heres the image

and now its happening when i simply leave the room. please fix the ice cave entrance area. if its not fixed then im gonna be stuck there
